docbook-html-forms software package provides Extension to DocBook XML for generating html forms, you can install in your Ubuntu 14.04 LTS (Trusty Tahr) by running the commands given below on the terminal,
$ sudo apt-get update $ sudo apt-get install docbook-html-forms
docbook-html-forms is installed in your system.
Make ensure the docbook-html-forms package were installed using the commands given below,
$ sudo dpkg-query -l | grep docbook-html-forms *
You will get with docbook-html-forms package name, version, architecture and description in a table
